Coordination of Care

After Diagnosis and referral to the Melanoma Program, our goal is that patients will not “get lost in the system.” From treatment through long-term follow-up, patients' progress will be closely followed by our Program Director and Melanoma Care Coordinator. That means, at any time, patients and their physicians have a single point of contact throughout the care continuum for treatment, education, and support. Patients will know up-front how to access information and that their custom care plan is managed by one group that is connected by a standard set of evidence-based treatment protocols (ETP’s).
