For physicians:

If you would like to refer a patient with melanoma to the North Bay Melanoma Program, please call Peter Brett, M.D., Program Director, or Patricia Grinnell, N.P., Melanoma Care Coordinator, at (707) 823-8565 or fax a request to (707) 823-7851. Nancy Carroll or Jan Winkel, Program Liaisons, will help arrange a medical consultation for your patient, and Dr. Brett and Ms. Grinnell will work with you to make sure ongoing care remains closely coordinated.

For patients:

If you have been diagnosed with melanoma in the past 2 years, you may refer yourself to the North Bay Melanoma Program (please check with your health insurance first, though, to see if your insurance requires your physician to refer you). Please call Peter Brett, M.D., Program Director, or Patricia Grinnell, N.P., Melanoma Care Coordinator, at (707) 823-8565 or fax a request to (707) 823-7851. Nancy Carroll or Amanda McLeod, Program Liaisons, will help arrange a medical consultation for you, and Dr. Brett and Ms. Grinnell will work with you to make sure ongoing care remains closely coordinated.
